Midler is the user of the shapeshifting Stand High Priestess (Stand), which has hard teeth which it can use to crush things easily. Initially, the wielder of the Stand was unknown, High Priestess being soundly defeated by Jotaro in Stardust Crusaders, leaving only an unconscious body for the Crusaders to find afterward.
Her current design and name (a nod to singer Bette Midler) first appeared in JoJo No Kimyou Na Bouken: Mirai E No Isan, the CAPCOM-produced CPS-3 fighting game. Upon being asked, Hirohiko Araki created her character to complement the game's roster at that time.
