The barrier jacket of Fate Testarossa used in Mahou Shoujo Lyrical Nanoha Detonation and Magia Record: Mahou Shoujo Madoka Magica Gaiden. It's the further enhanced version of Blaze Form II.
Blaze Nexus can also be included in this tag, it's a modified version of Blaze III near the end of Detonation.
